I was looking at the selection of books and I realized their were two spots between submarine 3 and mist groove 1,
1 of them is probably the shrine and the other is submarine 4?

yep, apparently there are two more submarine stages to come, one of them being the 2nd castle [Submarine Shrine]

it's not 100% sure, but i'd say about 99%.
go to youtube and search for the stick ranger hack video (v4.0 - hidden stages). and it's hidden in the gamecode.

...That's actually a good question. Hehe, if this were a platformer game there would probably be fluctuating waters, and the boss is only vulnerable when the waters recede. XD

nobody knows for sure, but based on the the dummy stage it seems to be unterwater.

There are already references to two further stages in the gamecode, "Submarine Shrine" and "Desert 1". Both are in the gamecode since 3.9, but don't contain real levels. The dummy stage of submarine shrine can be seen in the hacked video on youtube, desert1 isn't playable (right know, I remember it was playable as a dummy level too, but it isn't since at least 4 versions).

has anyone been able to beat the boss in submarine shrine. I just keep getting frozen and die.
